Position: Registered Nurse Per diem ***New Graduates Welcome*** Location: North Wales

1. Coordinate total patient care by conducting comprehensive health and psychosocial assessments in the home setting.
2. Perform admission, transfer, re-certification, resumption of care, and discharge of the home care patient.
3. Provide oversight for non-skilled staff members providing direct care to recipients in the home setting.
4. Must be willing to provide in-home nursing care (wound care, infusion services, medication education) to adults and manage patients.
5. Role is primarily field based with minimal time spent in the office.
6. Must be a graduate from an accredited School of Nursing.
7. Must be licensed in the state of Pennsylvania, as a Registered Nurse.
8. One, or more, years of experience, in a community/home health agency or in a home setting, is preferred.
9. Must have knowledge of Medicare and Medicaid guidelines.
10. Must be detail oriented.
11. Must have a criminal background check.
12. Must have current CPR certification. Online certification is not acceptable.
13. Must have a valid driver's license with current active auto insurance.
