An exciting opportunity has arisen for an Occupational Therapist working as part of the team in the Integrated Discharge Hub/UHW
About the job
Working within the Trusted Assessor model of practice the successful applicant will undertake end to end assessments, creating interim care plans for discharge and reviewing and right-sizing care packages in the community.
In addition, the Senior Occupational Therapist may have responsibility for the following:
Line Management and Supervision (both formal and informal) of staff at the following levels:
• Occupational Therapy assistant
• Senior Occupational Therapy Assistants
• Grade 7 Occupational Therapists
To be actively involved in business planning and take a lead role for an area of service development, as identified from the business plan each year, with the Team Manager through PPDR.

The successful candidate must also be able to work to strict deadlines and be extremely flexible. You must be able to work autonomously as well as being able to work as part of a team.
You will have excellent communication and organisation skills, and the ability to problem solve and provide outcome focused interventions.

It is essential that you have experience of working proactively with clients with physical disabilities and/or older people, hold a full valid driving licence and have use of a vehicle with business insurance.
Additional information
With effect from the 1st April 2021 this post attracts a market supplement of £3000 per annum full time equivalent.
This payment is reviewed on a 12 monthly basis.
