We are looking to recruit two dynamic and experienced Social Workers, with a passion and commitment for working within a community mental health service, to our team in Weymouth and Portland. We work in an integrated community team alongside health colleagues to support people with significant mental health needs, fostering a recovery model and a strengths‑based, person‑centred practice to promote independence and help individuals achieve their best outcomes.

What You Can Expect To Be Doing
You will undertake strengths‑based assessments, support planning and reviews to promote independence and work within a recovery model.
* Perform Mental Capacity Assessments
* Work with people subject to the Mental Health Act; complete S117 assessments and reviews
* Co‑locate with Community Mental Health Nurses, OT, Psychologists and Psychiatrists to provide delivery for adults with Serious Mental Illness or Autistic Spectrum Condition
* Collaborate with other health and social care professionals
* Identify carers’ needs and support their assessments
* Take leadership roles and support service development
About You
* A dynamic social worker with a strong value base and sense of person‑centred practice who can use a strengths‑based approach
* A demonstrated passion and commitment for working with people with significant mental health problems and promoting their recovery
* Excellent communication skills, ability to work in a fast‑paced environment, flexible approach, and capability to work closely with a range of professionals
* Creative and passionate about enabling people to be as independent as possible
* Confidence and good understanding of the Legal basis for practice, including Care Act 2014, Mental Capacity Act 2005, Mental Health Act 2007, Human Rights Act 1998 and Equality Act 2010
* Commitment to continuous professional development
* Embodies our behaviours of responsibility, respect, recognition and collaboration
* Able to fulfil the travel requirements of the post
Social Workers will need to be registered with Social Work England.

* Have a degree in Social Work or equivalent

Relocation Expenses Package
