The team is made up of GPs, ANPs, HCAs, pharmacists, and an experienced administrative team, all working together to deliver high-quality, patient-centred care.

Key Responsibilities:
* Management of long-term conditions (e.g. asthma, diabetes, hypertension)
* Childhood and adult immunisations
* Cervical cytology
* Wound care and dressings
* Health promotion and lifestyle advice
* Supporting QOF and other clinical targets
* Working closely with GPs, ANPs, and PCN staff to provide holistic care
