We’re now seeking an experienced and motivated Physiotherapist to join our multidisciplinary Therapy Team. This is an exceptional opportunity to make a meaningful impact by supporting individuals across our Discharge to Assess (D2A) pathway and wider care community to regain independence, improve mobility and experience a better quality of life every day. The Role As a Physiotherapist within our Therapy Service, you’ll play a crucial role in delivering high-quality assessments, treatment programmes and rehabilitation for residents in our 13-bed D2A Pathway 2 service, as well as for individuals living within the care home and retirement village. You will: Clinical Assessment & Rehabilitation Complete physiotherapy assessments for D2A Pathway 2 clients, permanent residents and retirement village residents Set, review and progress personalised rehabilitation goals and programmes Determine accurate discharge dates and post-discharge requirements, including equipment and care needs Order equipment through NHS community equipment services (e.g., Medequip) Conduct home visits to assess functional needs and support safe discharge Implement therapy treatments including exercise prescription, education, mobility practice and equipment recommendations Support residents in managing acute and long-term conditions, promoting movement, confidence and independence Multidisciplinary Collaboration Attend MDT meetings, sharing clinical insights and contributing to collaborative decision-making Work closely with Physiotherapists, OTs and support staff to deliver coordinated, resident-centred rehabilitation Engage with families, external services and clinical partners to ensure seamless and informed support Clinical Responsibility & Delegation Delegate appropriate therapy tasks to Therapy Assistants, ensuring safe, effective and well-supervised practice Maintain accurate documentation in line with professional, legal and organisational standards Communicate clearly, compassionately and confidently with residents, families, colleagues and external professionals Take an active role in developing and improving the Therapy Service, contributing ideas and supporting service-wide initiatives About You You’ll bring clinical confidence, professional curiosity and a commitment to supporting older adults to live well. You’ll thrive in a team environment, communicate with warmth and clarity, and approach rehabilitation with determination, empathy and creativity. Essential Skills & Experience: BSc (Hons) Physiotherapy with current HCPC registration and CSP membership Minimum of two years’ post-graduate physiotherapy experience, including work with older adults, frailty and dementia Strong assessment and rehabilitation planning skills, with experience in discharge planning, goal setting and community/home-visit practice Confident IT user with accurate record-keeping skills and experience supervising or delegating to therapy assistants Excellent communicator with empathetic, collaborative and person-centred approach Desirable Skills & Experience Experience working in care homes or geriatric rehabilitation settings Knowledge of BNSSG systems, D2A pathways, and equipment ordering platforms such as TCES Community Additional clinical expertise such as orthopaedics, neuro rehabilitation, wheelchair/seating assessment, or BWCS referrer accreditation Experience working in community rehabilitation, especially within North Somerset Demonstrated ability to support complex cases within multidisciplinary settings Full UK driving licence and access to a vehicle for work Why St Monica Trust?
