Overview
The successful candidate will be a Registered Nurse from either Acute & Community or Mental Health background who is keen to develop their assessment skills further in an autonomous role within the NHS Long Term Care Service. This role emphasises multidisciplinary working and requires a clear understanding of Continuing NHS Healthcare & NHS Funded Nursing Care policies, the associated financial implications, and the Social Services and Well-being (Wales) Act 2014.

A new Long Term Care Pathway supports secondary care with Continuing NHS Healthcare and NHS Funded Nursing Care processes, ensuring smooth transitions from acute episodes through discharge planning to Nursing Home placement.
Responsibilities
* Assess and monitor individuals in the Nursing Home sector; provide training and support to Nursing Homes.
* Review and assess patients for Continuing NHS Healthcare and Funded Nursing Care.
* Manage MDT meetings relating to funding and discharge planning.
* Work as part of a team and autonomously; monitor care standards within the Nursing Home setting.
* Support patients and staff as part of the Nursing Home Support Service and the Long Term Care Pathway.
Qualifications and Skills
* Enthusiastic, experienced Nurse with excellent assessment and communication skills.
* Ability to work as part of a team and autonomously.
* Clear understanding of policies and procedures related to Continuing NHS Healthcare, NHS Funded Nursing Care, and the Social Services and Well-being (Wales) Act 2014.
* Welsh language ability is desirable; English and/or Welsh speakers are welcome to apply.
Location and Travel
Base can be in Carmarthen, Llanelli or Pembroke Dock, with travel across the Hywel Dda University Health Board area and neighbouring counties.
