We welcome your application to join our rehab and recoveryhub at Wharfedale Hospital, Otley, Leeds, providing therapy support with theregistered therapists. Home first is the ideal outcome for many patientsleaving hospital, but a temporary stay in a setting that is a homelyenvironment that can provide 24-hour care is sometimes required. The rehab andrecovery beds do this, enabling early discharge from hospital and reducinghospital admissions. The rehab and recovery hub service delivers complex carewhilst empowering patients as part of a wider multi-disciplinary team. When youjoin us, we will work with you to develop your own personal developmental planthat supports your learning style. We provide training, clinical supervisionand access to a peer support network, we will enable you to develop yourclinical and leadership skills. The rehab and recovery hub service works on ashift pattern and provides a 7 day a week service.
Our MDTs will deliver daily evidence-based therapeuticinterventions, specifically designed to support people reach their maximumdependency state, using therapy outcome measures to demonstrate our success inachieving this. Our ethos is that the rehabilitation, recovery and reablementof service users is everybodys business.
Main duties of the job
We are looking for a person to provide therapy support tothe registered therapists at Wharfedale Recovery Hub, Otley. The admission intothe hub will be on two pathways, Discharge to Assess or an Intermediate carepathway.
You will provide therapy interventions over 7 days a week,your role will be to work with patients to progress through the prescribedtherapeutic care plans.
Our Multi Disciplinary Teams will deliver dailyevidence-based therapeutic interventions, specifically designed to supportpeople reach their maximum dependency state, using therapy outcomes measures todemonstrate our success in achieving this. Our ethos is that the rehabilitation,recovery and reablement of service users is everybodys business. Working inCommunity rehabilitation suits people who can work independently and within thewider multi-disciplinary team. You will be resilient, motivated, and adaptableto working in an ever-changing environment and of course, a passion forproviding the best possible care. You will have excellent communication skillsand maintain a high standard of clinical work. You will ensure compliance withour policies and procedures, you will work in partnership with all members ofthe multi-disciplinary time and wider services.
