Health Care Support Worker

The closing date is 24 April 2026

Pioneer Wound Healing and Lymphoedema Centres is seeking skilled and motivated Health Care Support Workers to join our team.

Our vacancies are in our main clinics at Crawley and Eastbourne. Our core business is specialist wound care and lymphoedema and you will work with our clinic teams to deliver first class services to patients.

Hours will be Monday through Friday though some weekend working may occasionally be required. If you have experience of working in hospitals or community services, please apply as follows. You will be working with excellent team members and will be part of innovative services.


Main duties of the job

1. To ensure the highest standards, by complying with and implementing Pioneer Policies and Procedures, namely: Clinical, Corporate, Health and Safety, Information Governance and Human Resources.
2. To support all clinicians; to assist in delivering evidence based wound care.
3. To deliver assigned delegated tasks of wound care under the direction of the named clinician, for which appropriate training and competency have been undertaken.
4. To maintain clinical records using both manual and computerised systems, to report and evaluate patients progress/change.
5. To attend annual mandatory training and role specific training for agreed competencies.
6. To endeavour to ensure that each patients experience is a positive experience and that at the end of the patients experience within the Pioneer clinic, feedback should be encouraged and captured.
7. To ensure that all Pioneer Risk Management systems and processes are implemented to reduce risk to its lowest practicable level.
8. To ensure that they attend annual mandatory training and comply with Pioneer Risk Management systems.
9. To support the TVNC in gathering and analysing local service data, including audit, in order to meet contractual requirements and KPIs.


Job responsibilities

Job Purpose

To support registered staff in providing a comprehensive, evidence based tissue viability service for all patients including those in the community setting.

Key responsibilities and accountabilities

1. To ensure the highest standards, by complying with and implementing Pioneer Policies and Procedures, namely Clinical, Corporate, Health and Safety, Information Governance and Human Resources.
2. To support all clinicians; to assist in delivering evidence based wound care.
3. To deliver assigned delegated tasks of wound care under the direction of the named clinician, for which appropriate training and competency have been undertaken.
4. To maintain clinical records using both manual and computerised systems, to report and evaluate patients progress/change.
5. To attend annual mandatory training and role specific training for agreed competencies.
6. To endeavour to ensure that each patients experience is a positive experience and that at the end of the patients experience within the Pioneer clinic, feedback should be encouraged and captured.
7. To ensure that all Pioneer Risk Management systems and processes are implemented to reduce risk to its lowest practicable level; this includes raising risk awareness in the clinic, seeking risk and safety advice and escalating risks that cannot be managed locally to the Nurse consultant and Senior Management Team.
8. To ensure that they attend annual mandatory training and comply with Pioneer Risk Management systems; ensuring that Governance, Risk and Quality management is an integral part of their appraisal and staff development.
9. To support the TVNC in gathering and analysing local service data, including audit, in order to meet contractual requirements and KPIs.
10. Undertake additional duties that may be reasonably required within the scope of this position.

Clinical Responsibilities/ Assessment and care planning to meet health and wellbeing needs/ Provision of care to meet health and wellbeing needs

* To assist the clinicians in undertaking holistic assessments
* To act as patient and staff advocate through the application of ethical, legal and professional knowledge and skills.
* Plan and co‑coordinate with the clinic coordinator and Nurse Consultant the patients discharge arrangements
* Act as an advocate for the patient, providing them with information and directing them to the relevant support services where appropriate
* Respect peoples dignity, wishes and beliefs; involve them in shared decision‑making and obtain their consent before undertaking nursing procedures
* Promote a patient focused multidisciplinary approach to care in collaboration with carers, health care professionals and other agencies
* Ensure that treatments prescribed are carried out, and that multidisciplinary team members are informed of changes in the patients condition


Communication

* Promote and maintain effective communication.
* Use communication skills in order to convey information effectively to patients, carers and other staff, overcoming any barriers to understanding and providing support during distressing or emotional events
* Ensure patients are able to gain access to health and social care information and support relevant to their needs which enables patients to make choices about adopting a healthier lifestyle
* Ensure patient confidentiality is maintained
* Maintain professional boundaries and working relationships with patients and colleagues


Personal and people development

* Demonstrate the qualities of a good team player
* Educate patients and their carer(s) as required where a need has been identified
* Be appraised at least annually and contribute to own personal development plan (PDP) and be responsible for developing and sustaining own knowledge, clinical skills
* Take part in reflection and appropriate learning from practice, in order to maintain and develop competence and performance


Health, Safety and Security

* Take personal responsibility for promoting a safe environment and safe patient care by identifying areas of risk and following the Incident, Serious Incidents and Near Misses reporting policy and procedure
* Comply with Pioneer policies for personal and patient safety and for prevention of healthcare‑associated infection (HCAI); this includes a requirement for rigorous and consistent compliance with Trust policies for hand hygiene, use of personal protective equipment and safe disposal of sharps and disposal of all body fluids in accordance with universal precautions
* All staff must be familiar with, and adhere to, and have undertaken training in Pioneer child and adult protection procedures and guidelines, in conjunction with the Local Safeguarding childrens board (LSCB) and Adults safeguarding board (ASB) policies and procedures.


Service Improvement

* Ensure the implementation of nursing practice standards, guidelines, protocols, policies and care pathways.
* Deliver care based on current evidence, best practice and validated research when available with reference to Pioneer policies and guidelines


Quality

* Promote a happy and supportive environment
* Uphold quality initiatives that improve customer care and enhance the interface between staff, patients and visitors.
* Participate in audit processes for monitoring and reviewing nursing practice

* Use electronic patient records (EPR) and develop other computer literacy skills.
* Collect/record data accurately in accordance with professional guidelines and Pioneer Core Standards.

* Take personal responsibility for ensuring that Pioneer resources are used efficiently and with minimum wastage.
* In accordance with the Pioneer responsibilities under the Civil Contingencies Act 2004 to undertake work and alternative duties as reasonably directed at variable locations in the event of and for the duration of a significant internal incident, major incident or pandemic

Person Specification


Qualifications

* Experience in a healthcare setting
* Evidence of recent and ongoing learning
* Willingness to learn and achieve in the field of tissue viability and lymphoedema


Experience

* Relevant experience of working at Band 3 and or above
* Healthcare experience in Community or Hospital settings
* Experience in providing wound care preferable but not essential
