This position is for a Grade Approved Mental Health Professional, we would also welcome applications from Progressed Social Worker's (non AMHPS /Grade ).
This is a full-time temporary specialist post funded until March. The post is seconded from Adult Social Care to work exclusively with rough sleepers with mental health issues. The referral criteria is broad and customers can self-refer through drop-in sessions, which is a unique aspect and separates it from existing services.

An important element of this role is establishing a relationship whereby someone trusts enough to discuss and disclose information. Engagement with the most complex customers is an extremely slow process for many reasons. The Social Worker carries out mental health assessments, Care Act assessments, develops support plans, acts as a liaison and gateway into mental health services and develops more effective joint working practices with other rough sleeping services.
The caseload is high as they support the cohort of rough sleepers on the street, in hotels and in emergency accommodation. Often cases are people with dual diagnosis (severe mental illness and problematic drug and/or alcohol use) and/or people with both mental health and physical health diagnosis. Most of the referrals are vulnerable and concerning in terms of the complex nature of their presentation and multiple diagnosis which puts them at much higher risk of accidental death. There are many barriers for the customer group, so being an advocate is a significant part of the role.

We are looking for dedicated and highly motivated team players to join us as Social Workers working in line with the Care Act. Our Social Workers play a critical role in supporting people to develop creative and innovative solutions to meet the outcomes they want to achieve.

7. Experience working with people with Learning Disabilities and or Mental Health needs and an understanding of the policy context surrounding these areas of practice.
8. Understanding of the complex working areas of forensic case work
9. Understanding of complex health and care needs
You will be:
10. Have AMHP qualification grade (postgraduate certificate on master’s level)
11. Qualified Social Worker grade .
12. Experience of managing high risk, complex cases and safeguarding Adults.
13. Experience of working in a mental health, understanding the legislation that underpins the role.
14. Feel comfortable working autonomously and be self-motivating individual with the passion to make a difference in adult social care.
15. An awareness of importance of multi-disciplinary working and an integrated approach.
16. Understanding of services essential to meeting the needs of people with Mental Health needs.
17. Ability to communicate effectively with customers, carers and families.
18. Ability to cope positively with change and an ability to promote positive changes to make a difference.
19. Ability to carry out assessment and support planning in line with relevant legislation.
20. Resilient personal stress management.
21. Be required to undertake an enhanced DBS plus Adults Barred List (Essential)
