The Care Manager is responsible for coordinating the delivery of services to guests in collaboration with the facility’s team members.

Responsibilities
* The Care Manager monitors and documents the effectiveness of treatment provided, coordinates the admission process, and determines with the interdisciplinary team (IDT) appropriate post-discharge services.
* Coordinates and updates the Care Plan. Attends meetings if requested by the guest or deemed appropriate by the IDT.
* Participates in daily IDT meetings, providing clinical review as necessary.
* Completes assigned sections of Discharge Plan Instructions and reviews clinical post-discharge instructions with the guest upon discharge.
Qualifications
* Must possess, at minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.
* Must possess a current, unencumbered, active license to practice as an RN or LPN/LVN through the State Board of Nursing.
