This specific drug and alcohol worker role will involve supporting people that are accessing Shared Care via their GP, and conducting outreach. Our Shared Care provision is accessed by people who are stable with their drug/alcohol use and are being prescribed opiate substitute medication by their GP, and are seen and supported by our drug and alcohol key workers within a local GP surgery. We currently work from five Shared Care GP practices within Amber Valley. This role will also involve conducting outreach to people who aren\u2019t able to attend our office in Ripley, due to their multiple and complex needs. You will manage your own caseload of people presenting with drug and alcohol issues and will engage them in regular key work sessions. This will involve offering a range of harm reduction interventions and structured key working, helping engage service users in a care-planned and recovery focused way to attain improved health outcomes and achieve their personalised treatment goals. You will deliver 1-2-1 care-planned support, facilitating group work sessions as required, and will use appropriate models, interventions and approaches tailored to meet the individual needs of service users. Furthermore, you will support them in their individual, person-centred journey to build recovery capital by partnership working with other support agencies and local recovery projects and mutual aid groups. You will work closely with specialist teams within our service, including recovery connectors, employment workers, rehab workers, counsellors and clinical colleagues. Full role training and shadowing opportunities will be included in your induction.

We encourage and welcome applications from people of all backgrounds and believe it is important to include people with lived experience to ensure the needs of the people we support are represented. We are committed to creating an inclusive working environment where everyone is free to be themselves and we ensure equity of opportunity.
