Experienced GP Clinical Pharmacist PCN 8 role at NHS
Overview
The post holder is an experienced general practice pharmacist and qualified advanced practitioner, who acts within their professional boundaries, supported and working alongside a team of GreenLight pharmacists in general practice. In this role they will be supported by practice GPs and the GL@GP Director who will develop, manage and mentor them.
Responsibilities
* Provide clinical pharmacy and advanced practitioner appointment slots for the Practice, proactively and competently managing acute and long-term conditions, prescribing/referring where appropriate.
* Work as part of a multi-disciplinary team to deliver medicines optimisation, chronic disease management, quality improvement, and other enhanced services.
* Support practice staff with regards to prescription and medication queries, ensuring integration with community and hospital pharmacy to improve access to healthcare and GP workload management.

Qualifications & Experience
* Minimum of 5 years\' post-registration experience with at least 2 years in General Practice
* Certificate in Non-medical prescribing (IP)
* Evidence of further education in clinical pharmacy such as NHS Pathway Training for Practice Pharmacists or a diploma
* Qualified advanced practitioner
Desirable Qualifications
* Leadership qualification
* Education & Training qualification
* Membership of the Royal Pharmaceutical Council or working towards Faculty membership of the Royal Pharmaceutical Society
Experience (Essential/Desirable)
* Essential: Working as a pharmacist in General Practice full time (or equivalent) for more than 2 years as a Practice Based Pharmacist
* Desirable: Experience of leading pharmacy professionals, managing healthcare professionals, teaching & training, mentoring and coaching for change, clinical supervision
