A fantastic opportunity has arisen for a Registered Social Worker to join the North Community Wellbeing Team within Stoke-on-Trent Adult Social Care and All Age Commissioning, recently rated ‘Good’ by the Care Quality Commission (CQC).
In this role, you will manage a varied caseload, working under key legislation such as the Care Act 2014, Mental Capacity Act 2005, and other relevant legislation & guidance underpinning adult social care.

Responsibilities
* Conducting Care Act Assessments, reviews, and support planning
* Undertaking Safeguarding Enquiries and Mental Capacity Assessments
* Maintaining accurate and up-to-date client records
* Producing high-quality reports and demonstrating strong communication skills
