Offer description

Provide expert clinical support and leadership to community nursing staff working within theWindsor & Maidenheadservice.

Collaborate closely with the Operational Team Lead to ensure staff competency, including overseeing induction processes and ongoing clinical development.

Champion evidence-based practice to ensure delivery of high-quality, safe, and effective patient care.

Act as a senior clinical resource, offering enhanced support and guidance for complex patient cases.

Promote and support initiatives aimed at reducing inappropriate or unscheduled hospital admissions andfacilitatingtimelyhospital discharges.

Qualifications & Experience:

Specialist Community Nursing Qualification (DN SPQ).

Advanced History Taking qualification.

* Independent Prescribing qualification (V300) - completed or nearing completion.


Main duties of the job

Clinical Governance & Care Quality

Ensure care delivery is based on sound clinical assessment and robust care planning processes.

Monitor and review staff practice regularly tomaintainhigh standardsof clinical care.

Investigate complaints, safeguarding concerns, and serious incidents,identifyingroot causes and implementing improvements.

Add areas of concern to the risk register anddisseminatelearning across the team.

Leadership & Team Management

Exercise leadership and sound judgment insupportingthemanagementteam.

Support recruitment and retention strategies to build a resilient and skilled workforce.

Lead on staff induction, personal development, and professional growth.

Person Specification


Education/Qualifications/Training

* NMC registered Nurse
* Specialist Community Practice qualification relevant to field of practice (DNSPQ). (DN Degree, diploma or Certificate in District Nursing)
* MUST have Advance History Taking
* Non-Medical-Prescriber Qualification or nearing completion of V300.
* Active on the Mentor Register
* Evidence at studying at master's level


Continuous Professional Development

* Evidence of continuous professional development


Previous Experience

* Clinical leadership within a community environment
* Community Nursing experience
* Teaching/assessing in practice
* Able to conduct courageous conversations and support staff with performance related issues.
* Coaching


Additional Requirements

* Valid full UK licence and access to a car for wok purposes.
