Overview

An exciting opportunity has arisen for the role of a Community Care Navigator within Stockton Community Mental Health Services. The successful applicant would be based between Brook House and Wessex House in Stockton, with lots of community based work in people's own homes and also with our partner agencies across the locality. The post holder will work as part of the multi-disciplinary team delivering high quality patient focused care in a community setting. This role supports patients and carers to navigate services in their local community, throughout their journey within the mental health system. They will provide continuity of patient care via a bespoke step-up / step-down model which begins at the point of access, to support and promote recovery and wellbeing. This will improve the lives of people with mental ill health by minimising the impact of their condition through the delivery of care navigation in line with Community Transformation.


Requirements

You must have the Right to Work in the UK in order to be successfully appointed to this role. Please note, this role does not meet the required eligibility criteria for sponsorship under the Skilled Worker visa and therefore the Trust is unable to offer sponsorship for this particular role.


Responsibilities

* Regularly accompanies and provides support to patients in a variety of settings, as delegated and dependent on current risk assessment. This includes social inclusion, community integration and transfer between appropriate areas e.g. inpatient wards, community hub, group activities.
* Supports patients & carers to identify wellbeing needs and delivers low-level interventions accordingly, e.g. sleep hygiene, healthy lifestyle advice, and/or introduces patients to an appropriate colleague/service.
* Works as part of a multi-agency service in delivering mental health support alongside our partner agencies across Stockton, including Local Authority and VCSE.
* Where appropriate and with authorisation, acts as an advocate for patients/carers.
* Responsible for recognising the signs of patient harm, abuse or neglect, including poor clinical practice, identifying issues regarding the safeguarding of children and adults who may be at risk and reporting all such concerns in accordance with Trust Safeguarding procedures.
* Undertakes delegated activities within level of own competency, and with due regard to risk assessment. Demonstrate resilience in caring for patients that have a range of mental health conditions, who may demonstrate behaviours that challenge and may have difficult family situations of relevance.
* Completes and accurately maintains patient care plans & records.
