Job Title: Reablement Officer (Walsall Council)

Reference: SS05025

Grade: G4

Salary: £26,403 - £28,142 Pro Rata to hours worked

Hours: 30 hours per week

Position: Intermediate Care Service in Walsall, a multi‑disciplinary team operated jointly between Walsall Healthcare NHS Trust and Walsall Adult Social Care.

We are looking for enthusiastic and committed individuals to join our team to help shape and deliver our services, and contribute to a model of best practice.


Responsibilities

* Support customers in their own homes to develop or regain skills and confidence, delivering an active Reablement programme in accordance with individualised Reablement Plans.
* Support and enable activities of daily living (ADL’s) – including personal care – in partnership with other agencies, ensuring that religious, cultural and gender needs are met, and personal dignity and privacy are maintained.
* Work cohesively within a multidisciplinary team to maximise customer independence and reduce the need for ongoing support.
* Help customers enjoy full access to community resources, and develop informal support networks.


Qualifications

* Minimum qualification: NVQ Level 2 Health and Social Care or equivalent.
* Experience working in adult social care and in community‑based Reablement Services, both in customers’ homes and in a multi‑disciplinary setting.
* Understanding of strength‑based working and a commitment to safeguarding children, young people and vulnerable adults.
* Excellent written and verbal communication skills.
* Enhanced DBS clearance (must declare any overseas periods and provide required documentation).
