Permanent contract – £32,320 to £33,868 per annum (Scale J), 30 hours per week – Social Worker (North Norfolk & Broadland). Service based in Kelling Hospital.
About the role
* Provide an integrated Health and Social Care approach to assessment, support and care planning in the community.
* Collaborate with multidisciplinary teams, including the Hertfordshire Partnership NHS Foundation Trust, to deliver specialist learning disability support to adults.
* Build strong partnerships with service users and their families to identify positive outcomes and progress towards independence.
* Serve as an active member of the Northern Learning Disabilities Team, contributing to service improvement and learning activities.
Key responsibilities
* Conduct assessments and develop welfare plans for adults with learning disabilities.
* Deliver person‑centred support and interventions in the community, coordinating with health and social care providers.
* Maintain accurate, timely and confidential records of assessments, progress and outcomes.
* Engage with service users, family members, carers and professionals to facilitate partnership working.
* Advocate for the rights and well‑being of service users, ensuring effective care provision.
What we’re looking for
* Qualified Social Worker (current professional registration).
* Experience working within Health & Social Care teams with adults with learning disabilities.
* Excellent communication and interpersonal skills, with the ability to build trust and partnership.
* Strong organisational and prioritisation abilities, able to work under pressure and meet deadlines.
* A proactive, creative and solution‑focused approach to service delivery.
