About Connect to Community

We've recently restructured and are launching an exciting new way of working — and this is your chance to be part of it from the very start. You'll have real influence in shaping practice, building strong team cultures and helping us deliver a values-driven, strengths-based service that truly puts residents first. Our new operating model brings together Connect to Community (C2C) and Area Teams, creating a seamless journey for residents:

* Our C2C Teams act as the first point of response, providing urgent assessments, safeguarding, and support planning.
* Our Area Teams offer continuity for those with longer-term or complex needs, focusing on proactive case management and person‑centred care.

Our new operating model is designed to cut down on unnecessary bureaucracy and increase meaningful time with people in their communities, focusing on what really matters: listening, building relationships & creating solutions that work.

We're proud to offer flexible working, giving you the freedom to work in ways that suit your life while staying connected to supportive, high-performing teams. Everything we do is guided by our values — collaboration, respect and ambition for our residents — and we're looking for people who share our values and are motivated to make a significant and positive difference to the lives of our residents. If you want to be part of a service that's growing, evolving and committed to doing things differently, this is the place to build your next chapter.

About The Role

As a Carers Practice Advisor within C2C, you will play a vital role in promoting high‑quality, consistent carer practice across Adult Social Care, ensuring this is aligned with the Care Act 2014 and underpinned by a whole‑family approach. In this role, you will act as a subject matter expert in both carer and young carer practice, making sure that the voice of carers remains central to assessment, planning and review processes. You will undertake direct work where required, including completing carer assessments and reviews, and will provide advice, guidance and professional support to operational colleagues across services.

You will also support consistency and quality of practice through practice development activity, reflective sessions and audits, and will contribute to the design and delivery of training to increase carer awareness and confidence across teams. Working closely with the Senior Carers Practice Development Advisor, you will be involved in projects, service improvements and cross‑organisational initiatives, while also building strong partnerships with internal teams and commissioned carer services.

As part of C2C, you will additionally contribute to shaping how carers are identified, supported and responded to at the earliest point of contact, helping to ensure timely, effective and person‑centred support.

Shortlisting Criteria

In order to be considered for shortlisting, your application will clearly evidence the following skills and align with our behaviours :

* Have significant experience working with carers and/or young carers
* Have a strong understanding of the Care Act duties towards carers
* Can confidently provide practice guidance and professional challenge
* Are skilled at building relationships across teams and partner organisations
* Are confident delivering training or facilitating reflective practice
* Are motivated to improve consistency, quality and outcomes for carers
