Job summary
An exciting and rewarding opportunity has arisen for a suitably qualified, enthusiastic and passionate Primary Care Nurse to join the Bridgend East Cluster.
The East Cluster includes five GP surgeries, covering a population of approximately 83,689 patients and works collaboratively alongside other Primary Care Contractors such as community pharmacies, Dental and Optometry colleagues, District Nurses, the Third Sector, other Allied health care Professionals and the Local Authority to support the health of our population. The cluster is fully committed to meeting the health needs of the population it serves and by working collaboratively with a 'multi-disciplinary team' approach allows us to achieve our goals.
The Primary Care Nurse will work within the cluster multi-disciplinary team, providing a domiciliary service for the co-ordination, management and monitoring of the care of the housebound patients with long-term chronic illnesses (heart disease, COPD and diabetes).
The emphasis of this role will be on service quality, through the optimisation of care where multi-morbidity exists, with the aim of maximising wellbeing, self-empowerment, quality of life and mobility.

Main duties of the job
The range of assigned tasks will be carried out with the highest standard to support and enable qualified staff to provide patient care, which reflects the quality of standards and mission of the HB. The post holder will be required to carry out a range of assigned tasks involving direct and indirect care for the benefit of the patient/client in accordance with agreed protocols. The range of responsibilities will involve rotation as appropriate to the needs of the service and will include the provision of clinical sessions across a number of chronic conditions including all the patients within care homes. The post holder will be responsible for ensuring that all policies and procedures are observed, and meet the HB requirements contained in the Health and Safety Policy. Will also be expected to assist in development of policies relevant to speciality.
The post holder has the freedom to act within established parameters, work is managed rather than supervised. The post holder is required to act independently within HB and professional guidelines, referring to supervisory management as appropriate.
This post is fixed term for 24 months due to meet the needs of the service.

4. Registered Nurse
5. Post registration qualification/training to diploma level within the field of chronic conditions management
6. Venepuncture training
7. Ability to demonstrate responsibilities under NMC Code of Conduct Working knowledge of multidisciplinary team working Working knowledge of clinical governance.
Desirable
8. Diabetes, Respiratory or CHD Diploma or equivalent
9. Specialist Practitioner Qualification in Practice Nursing / District Nursing
10. Knowledge of audit tools
EXPERIENCE
Essential
11. Substantial post registration experience within Primary Care encompassing all aspects of practice nursing chronic conditions management, public health nursing and first contact nursing
12. Organizational, interpersonal and operational experience needed for post management of resources and the assessment, planning and delivery of direct patient care.
13. Proven record of Implementing innovation and change in clinical practice
14. Audit and/or research experience
