Relief Hub Facilitator Hourly Rate £15.56 - 17.47 per hour Type of vacancy Relief Work pattern Relief Hours per week As and when required Work Base Broxtowe Community Hub, Nottingham, NG9 4FR andGedling Day Service, Nottingham, NG5 8AA Working Arrangements Office Based Job Description / Person Specification Competency Framework Number of posts 1 Interview date 25 June 2026 Closing Date 31 May 2026 23:55 We are seeking dedicated and compassionate Relief Hub Facilitators to join our North team. This role has responsibility for ensuring the day opportunities are meeting the care and support needs for people within our practice framework. Hub Facilitators work within a geographical area and must be prepared to work at different buildings across the district. The role involves providing high-quality physical and emotional support to individuals, whilst respecting privacy, dignity and specific needs. The team works within a risk management framework, ensuring their personal welfare and ensuring escalation of any risks. You will have responsibility for developing care plans based on effective consultation with people, carers, other relevant agencies and meet individuals' care and support needs appropriately and respectfully. You will work with people on a 1:1 basis and within groups in the Hub and out in the community. You will use any equipment as directed by the care plan, following appropriate training and are expected to report any health and safety issues relating to premises, vehicles, equipment, and other facilities used by day opportunities. You will ensure effective delivery of day opportunities activities within departmental policies, legislation, and practice standards and Support individuals to stay in control, promoting, developing, and maintaining independence. You will create and review strength-based care plans and risk assessments, with input from the people accessing the Hub, carers and other professionals. You will undertake medication administration and recording procedures in line with the medication policy. You will be responsible for creating safe moving and handling plans, and will be required to undertake these duties as required. You will respond appropriately to crisis and emergency situations, and escalate incidents following protocol. You will collaborate with stakeholders and contribute to reviews, meetings, and other forums as required. This role also involves effectively supervising Hub Enablement workers. You must have: A Social Care qualification or equivalent, with a care qualification or working towards Level 3 Diploma in Adult Care (with a supervisory role). Knowledge of relevant legislation. Evidence of continuous mandatory training and development. Knowledge of main ICT systems in the service area. Excellent professionalism, efficiency, and effectiveness in service delivery. Ability to collaborate well with colleagues and work independently. Effective communication skills with individuals at all levels. Experience of working with people with disabilities or in care settings. Experience of empowering and advocating on behalf of individuals. Experience of handling money and understanding financial guidelines. Experience of supervising staff and delivering a wide range of day-to-day opportunities. You must have a minimum of two years' experience in service delivery of a care setting. Have experience in empowering and advocating for individuals.
