Your role : The Community Adult Front door is a new and forward-thinking team built on a multidisciplinary approach to the effective triage and management of referrals for adult social care support. In this role you will have responsibility for the new Community Adult Front door team (CAF) embedding an asset-based approach in ensuring our residents receive the right support at the right time, with a focus on early intervention, prevention, independence, and safety. You will ensure effective and efficient day to day operation of the service, its development and performance, alongside the wellbeing of the team. You will also be responsible for the direct line management of key, identified professional roles. About you : We re looking for someone who has a management qualification at NVQ Level 5 or equivalent managerial experience, with a willingness to undergo NVQ Level 5 qualification. Driven and confident at working in a fast-paced environment, you will have experience of managing a team with a proven ability to lead and motivate others. You will understand budgetary impacts and have the ability to work in partnership to develop the trust and co-operation of a broad spectrum of partners and colleagues. Occupational Therapy degree or diploma and registration with HCPC is desirable or experience of working with therapists/Early Intervention services, as well as social care teams. The multi-disciplinary team consists broadly of Occupational Therapy, Social Care, Reablement and Assistive technology staff, as well as Customer Service Advisors and Coordinators who handle calls/screen and prioritise referrals. The team aims to respond to urgent need and to maximise opportunities for independence, prevention services, information advice and signposting. This is an exciting opportunity to play a key role working as part of a multi-disciplinary team, promoting a joined-up approach within the team and with wider partners to achieve the best outcomes for our residents.
