About the role The Residential In-Reach service is a Department of Health funded program aimed at reducing avoidable presentations to the Emergency Department and admissions to hospital, by residents in aged care facilities through timely and appropriate care provided in their homes. The CNC offers a “proactive, preventative” response approach for residents in partnering residential aged care settings, who are deteriorating and experiencing worsening health symptoms. In collaboration with, the resident, family, the residents General Practitioner (GP), and Facility RN’s, will contribute to a preventative management plan based on comprehensive evidence-based assessment’s addressing specific health risks associated with ageing. Recognition of clinical deterioration in the older person and escalation of care to the GP, RiR Nurse Practitioner, Geriatrician, or other supportive services to maintain patient safety and optimise health outcomes. Clinical triage of residents in line with RiR referral and treatment pathways. Ensures established care plans and treatments are performed, documented, and communicated to all stakeholders including the resident, their family, the Residential Aged Care Facility (RACF), the GP, the hospital if appropriate and other engaged supporting agencies. Facilitate streamlined transitions to and from clinical settings. Ability to work collaboratively and cohesively with colleagues, supervisors, and other stakeholders Authorised to practice as a Registered Nurse with the Australian Health Practitioners Regulation Agency (AHPRA) together with demonstrated ability to meet the ANMC competencies for the Registered Nurse. 5 years full time equivalent post registration experience with at least 3 years full time equivalent experience within the specialty or related streams. A background and or post graduate qualifications in emergency, wound, continence, and gerontology nursing is highly desirable.
