Job responsibilities
Principle Clinical Responsibilities:
* Meeting patients nursing care needs as identified, or where care has been transferred to a GP. This will usually be in surgery but occasional home visits will also be required.
* Initiating and maintaining health screening and promotion clinics for identified areas of the practice population, recognising where modification of systems or referral is necessary.
* Managing the nursing care of patients with chronic disease, employing personal recall systems suitable to the practice needs. This includes using the practice computer systems for input of information for recall and screening purposes.
* Initiates the process of diagnosis with patients suspected to have a chronic disease (e.g. diabetes, COPD, asthma, CHD) referring to other clinicians as appropriate when adequately trained. Undertake medical reviews with patients.
* Administers adult and child immunisations and vaccinations in accordance with national and local programmes. Is competent in anaphylaxis and resuscitation techniques. Ensures safe storage, rotation and disposal of vaccines and drugs and adequate stock levels are maintained.
* Offers comprehensive travel health advice for patients prior to travel including malaria prophylaxis, safe sex, food hygiene, sun protection, first aid and emergency medication, health insurance and vaccinations, where competency allows.
* Undertakes procedures including venepuncture, glucose tolerance tests, BP management, urinalysis, peak flow, spirometry, new patient health checks, pregnancy testing, stitch/clip removal, ear syringing and cytology, wound management, and cryosurgery when adequate training achieved. Has an interest in women’s health and a willingness to develop in coils and implants.
* Recognises and understands the importance of observing, documenting and communicating signs and symptoms of child and adult abuse.
* Works within all relevant practice policies and procedural guidelines (e.g., infection control, chaperoning, patient group directions).
* Refers to other members of the extended primary health care team.
Professional Responsibilities:
* Attend and contribute to relevant internal and external meetings including practice clinical meetings and nurse team meetings.
* Maintain a personal development and training plan to stay up-to-date with current thinking and developments and identify training needs.
* Promote evidence based practice through use of the latest guidelines.
* Maintain professional registration and work within the latest NMC Code of Professional Conduct.
* Record accurate consultation data in patient records in accordance with NMC guidance and standards.
* Keep up to date with pertinent health policy and work with the practice team to implement strategies.
* Work collaboratively with colleagues internally and externally.
* Demonstrate leadership and promote the role of specialist nursing expertise within the practice.
* Contribute to practice audit and research activity as required.
* Promote effective communication, relationships and team working within the practice.
* Participate in clinical supervision for development purposes.
Personal Responsibilities:
* Keep up to date with practice policy and procedures relating to this post.
* Attend relevant training courses as requested by management.
* Help the practice develop in a cost effective manner, managing resources such as stock control of dressings and vaccinations.
* Understand contents of the Staff Handbook.
* Utilise up to date information technology to collate accurate information as required by the practice.
* Make recommendations to raise practice standards and service delivery.
* Observe confidentiality at all times and conduct yourself professionally.
* Attend annual IDPR with the Practice Manager.
* Stay informed about data protection issues relating to the post.
* Report and record accidents in line with the Health and Safety at Work Act 1974.
