Hazelmead House is an established housing project for people recovering from mental health issues, located in Swindon's old town close to all amenities and the town centre.
Role Summary
We are looking for a compassionate, proactive, and energetic Support Worker to join our team and deliver person‑centred, strengths‑based support to our residents. This permanent part‑time role (30 hours per week on a 4‑week rolling rota, including sleep‑ins) involves helping residents take meaningful steps toward independence.
Responsibilities
* Play a vital role in helping residents achieve their goals, strengths, and aspirations, building confidence and purpose
* Manage a caseload of residents, guiding them through referrals, interviews and assessments with empathy and professionalism
* Act as the key point of contact after they join the service, welcoming them, building trust, and delivering tailored support through one‑to‑one and group sessions
* Focus on outcomes, empowerment, and helping people realise their full potential
Qualifications
* Proven experience working with vulnerable people with support needs
* Proven experience of caseload management, with a clear focus on outcomes for customers and their wider community
* Ability to demonstrate partnership working with specialist agencies such as mental health teams, offending services or other relevant services
* Excellent communication skills, with the ability to work in a professional manner with a number of different agencies, including representation at multi‑disciplinary meetings
* Ability to work positively and creatively with support needs
