Nurse Consultant in Renal Transplantation
The lead nurse will provide evidence-based services through regular literature review, British Transplant Society & NICE guidelines, and update Trust/Transplant departmental guidelines and protocols to ensure high quality care. They will independently undertake complete clinical assessments (in accordance with RCN competencies for the advanced nurse practitioner, 2008) of transplant patients in both routine and emergency situations, using their judgment to establish management plans through assessment and diagnosis. The role includes telephone triage, managing a defined caseload of patients with acute/chronic transplant disease, and overseeing the management of waiting lists and regional transplant assessment clinics.
The nurse will lead the delivery of nursing care to transplant patients across the Trust, promoting best practices and leading change management strategies to enhance care. They will request and interpret laboratory and radiological investigations, initiating appropriate treatments, and serve as an expert resource supporting colleagues in primary and secondary care. The role involves developing protocols for nurse-led and regional services, fostering clinical expertise in kidney disease, and line managing junior team members, including appraisals.
Effective communication with patients, families, and carers is essential to empower informed decision-making and promote health. The nurse will review and develop the transplant nursing service structure, lead a five-year service strategy, and contribute to clinical governance, risk management, and cost-effectiveness strategies. They will develop patient and public involvement methodologies, manage the renal transplant budget, and participate in trust and regional committees and meetings.
Additional responsibilities include incident reporting, risk assessment, staff training, recruitment, retention, disciplinary procedures, policy review, clinical audit, and participating in national policy setting. The nurse will support inter-professional education, maintain current knowledge of transplantation advances, and disseminate information through various media. They will prepare for appraisal and professional development, participate in on-call duties, coordinate referral processes, and ensure confidentiality and safeguarding policies are followed.
