Detailed job description and main responsibilities

1. To provide highly specialist nursing advice underpinned by theory and experience to support
patients and their families/carers
2. To plan, develop, improve and promote the service ensuring the delivery of high quality, cost
effective care
3. To act as an autonomous practitioner, independently planning, implementing and evaluating
programmes of care achieved through systematic and holistic assessment, clinical
examination skills, personalised care planning and independent onwards referral and
discharge to primary care team
4. To provide patients and staff with highly developed specialist advice to manage complex
symptoms
5. To develop and share expertise within the multidisciplinary team
