Occupational Therapist - Advice and Wellbeing Hub

We are looking for a motivated and talented full-time Occupational Therapist to join the Advice and Wellbeing Hub within Adult Social Care. About The Role
As an Occupational Therapist in the Advice and Wellbeing Hub, you will be part of a friendly and supportive team. You will demonstrate strong communication skills and the ability to build and maintain relationships with colleagues, service users, and health care providers. The hub takes new referrals into Adult Social Care for people from 18 years upwards and aims to promote wellbeing and independence through skills support, Technology Enabled Care (TEC), equipment, and short-term interventions.
You will be based in the Advice and Wellbeing Hub OT Pathway, working with other Occupational Therapists and Social Care Coordinator colleagues to assess adults with a broad range of needs. You will hold a varied caseload, contribute to team performance and positive outcomes for residents, and support the teams Duty function by triaging cases. You will work within the legal framework provided by statute, guidance, policies, and procedures.
Responsibilities
* Conduct strong, strengths-based occupational therapy assessments and interventions with service users in their own homes to help them develop, recover, or maintain meaningful occupations.
* Provide equipment, adaptations, moving and handling support, and goal planning to promote wellbeing and healthy occupations.
* Work closely with care providers, Housing Services, and Health colleagues; collaborate with Reading Borough Council colleagues and other partners to achieve outcomes.
* Support the hub's duty function by triaging cases and ensuring timely responses.
* Operate within relevant statutory, policy, and procedural frameworks.
What You Will Need To Have
* A degree in Occupational Therapy
* Registered with HCPC
* Community experience is ideal
