Are you an experienced Practitioner looking for a change of role? If so then we would welcome your application. We have a full time Social Worker vacancy in the Eastbourne and Hastings Community Hospital Team. This role is predominantly in the Eastbourne side of the team however, there may at times be a need to travel outside of the Eastbourne area depending on the needs of the service. You will need to be a qualified, registered (with Social Work England) and experienced Social Work professional. To carry out the role you will need a detailed knowledge of the Care Act (2014), Mental Capacity Act (2005), Equality legislation and the Deprivation of Liberty safeguards. You must be able to implement this legislation in your daily practice. You will also be expected to carry out the Local Authorities safeguarding duties to protect and mitigate risks to the individuals we serve. Experience of Court of Protection work would be an advantage in this role; however training is offered to successful candidates. We have excellent relationships with local partners and hospital trusts. The team are responsible for undertaking assessments of need for individuals discharged from an acute hospital back to their home or into a discharge to assess bed. Where necessary, we work with our local Integrated Commissioning Boards (ICB\'s) to ensure people we serve get access to continuing care funding. We are unique as a service in the fact that we have case responsibility for people who are inpatients and for those who have been discharged in the community requiring a Care Act assessment of need. Responsibilities
* Undertake safeguarding duties under local authorities to protect and mitigate risks to the individuals we serve.
* Carry out Care Act assessments of need for individuals discharged from acute hospital to home or for discharge to assess bed.
* Implement detailed knowledge of the Care Act (2014), Mental Capacity Act (2005), Equality legislation and Deprivation of Liberty Safeguards in daily practice.
* Collaborate with local partners and hospital trusts; work with Integrated Care Boards (ICBs) to access continuing care funding where required.
* Travel outside the Eastbourne area as needed to meet service requirements.
* Experience of Court of Protection work is advantageous; training is provided to successful candidates.
