An exciting opportunity is available within Adult Care, Housing and Public Health, as an experienced Social Worker working in our Enablement Assessment Team.
About the role
As a Social Worker in the Enablement Assessment Team, you will undertake Care Act Assessments to support customers to remain in their own homes as independently and safely as possible and facilitate safe and timely discharges from bed bases such as intermediate care beds and short‑stay placements within a 24‑hour care setting.
You will screen safeguarding concerns, complete initial safeguarding enquiries under Section 42 of the Care Act, and focus on safety planning, risk management and working with partners including the Police, NHS professionals and Housing colleagues.
Supporting customers to maximise independence and personal outcomes will involve providing comprehensive advice, information, practical support and signposting, focusing on strengths, prevention, early intervention and the promotion of wellbeing.
You will work as part of a wider team, closely collaborating with Wellbeing Advisors, Assessment and Review Coordinators, Occupational Therapy colleagues and Team Managers.
Responsibilities

Qualifications and characteristics
* Compassionate, innovative and resilient social worker with a drive to deliver improved outcomes.
* Experience of applying statutory legal frameworks and policies including the Care Act 2014, the Mental Capacity Act 2005 and Making Safeguarding Personal.
* Confidence in working at pace in challenging situations and making sound decisions quickly to support those in crisis.
* Experience working with health colleagues, voluntary sector organisations and partner agencies to maximise independence and resilience.
* Initiative and ability to respond independently to unexpected situations.
