The post holder will work as part of the Police Pathways Team (Health Based Place of Safety, and Joint Response Vehicle) to undertake allocated duties under the supervision of the Police Pathways coordinator within the team.

Use of a car or access to ameans of mobility to travelacross the Trust footprint in linewith service needs is a requirement of this role

The post holder will work alongside mental health practitioners, supporting patients, who are referred into the Police Pathways Team and ensuring the highest possible standard of care in accordance with the policies and procedures of Pennine Care NHS Foundation Trust.To support the Police Pathways Coordinator as part of the MHJRV service to provide an effective and efficient response to referrals from GMP and NWAS.

To support patients to access the right care at the right time in the least restrictive way, reducing system pressures across emergency services, our locality EDs and S136 suites.


Main duties of the job

To provide support to Police Pathways practitioners in ensuring all patients referred into the team are cared for in line with Pennine Care NHS Foundation Trust policies and procedures

o To assist qualified practitioners in assessment, planning, implementing and evaluating assessments of patients under a section 136o To ensure that patients access appropriate services.o To provide support and interventions to patients in the HBPoSAFC 27/1/25 JME-220-24 Health Care Support Worker Band 3o To sign post patients to third sector provision as appropriate other community resources such as Citizens Advice Bureau, Peer Support Service and other resources found in the Local Borough Signposting Directory.o Provide regular and practical support for patients and their carers' in managing dignity and independence.o Assist patients to gain access to resources required to meet their needs.o Provide information on health promotion and support physical healthmanagement.o Report regularly to other members of the multidisciplinary team

Job responsibilities

Person Specification


Education / Qualifications

* Good standard of general education including good numeracy and literacy skills
* NVQ 3 or equivalent
* Commitment to undertake Continuing Personal Development
* Evidence of recent further development / training.


Experience

* Experience of undertaking a supportive role
* Experience of working with individuals with mental health needs
* Experience of working with individuals in distress
* Experience of lone working


Knowledge

* Understanding of mental health issues and how these impact on individuals.
* Understanding of confidentiality and data protection issues.
* Knowledge of physical health observations.
* Awareness of local services
* Knowledge of benefits / employment systems.
* An understanding of the mental health system


Skills and Abilities

* Ability to listen effectively
* Good written and verbal communication skills
* Ability to empathise with service users
* Ability to implement care plans for individuals with the support of others as appropriate.
* Ability to create innovative solutions to help empower service users
* Ability to work unsupervised in a range of settings
* Ability to form positive therapeutic relationships with service users and carers.
* Willingness to make a positive contribution to improving the quality of life for people with mental health problems.
* Ability to acknowledge diversity and promote anti-discriminatory practice / equal opportunities.
* Ability to provide practical support with daily living activities
* An ability to act calmly in emergencies and to respond in a professional manner to stressful and challenging behaviour
* IT Skills
* Ability to work flexibly and to prioritise workload

* Willingness to engage with or have undertaken Venepuncture and physical health checks training.
