Job summary


Are you ready to make a real impact in your community? We are looking for a passionate and proactive Health Care Support Worker to join our Urgent Community Response Team, a fast paced and dynamic service that helps keep people safe at home and out of hospital.

In this hands on role, you will be right where you are needed most, face to face with patients, supporting their recovery and wellbeing. Every day brings new challenges and opportunities to make a genuine difference in someone's life. You will be part of a close knit and supportive team that values compassion, teamwork and high quality care.

What we are looking for is a caring and empathetic individual who thrives on helping others. Someone who can stay calm and focused in a fast moving environment. A strong communicator and team player. Previous experience in healthcare is desirable, but your passion and attitude matter most.

Main duties, tasks & skills required


Maintain excellent communication with patients, relatives and members of the team regarding all aspects of care, demonstrating a variety of communication skills in accordance to patient need.

Maintain clear, concise and legible documentation in accordance to organisational policy.

Communicate on complex and sensitive issues.

Use a range of verbal and non verbal communication tools to communicate complex information effectively with service users, families, colleagues, team members and other agencies to facilitate the delivery of care programmes, to promote effective communication.

Reflect on barriers to communication and use knowledge of different methods to support and improve communication with individuals.

To communicate appropriate information to and from other departments and service areas as required.

Qualifications



Essential


Full driving licence required.

Experience working within a health care setting.

Literacy and numeracy at Level 2 (GCSE grades A to C or equivalent).

Care certificate, First aid certificate, Community experience

Experience in carrying out clinical skills, for example blood pressure and urinalysis.

Previous experience in group work, such as conducting patient education sessions.

Demonstrates an acceptable level of health knowledge.

Awareness of risk management.

Knowledge of multi agency services and referral processes.

Able to work effectively as part of a team.

Flexible and adaptive approach.

Non judgmental attitude.

Self reflective regarding personal and professional development.

Approachable and person centred in interactions.

Able to secure cooperation of colleagues across agencies.

Capable of working without direct supervision.

Practical experience in a health care setting before, during or after education.

Able to manage a small caseload.

Acts within own limitations and seeks advice from trained staff to recommend or decide on the best course of action.
