As a Social Worker, you will work as part of a multidisciplinary team, providing high-quality social care services to adults of all ages experiencing mental ill-health. Your core responsibilities will include assessment, care planning, and coordination of social care from a social perspective for adults accessing the service. You will deliver recovery-focused care and support, emphasizing choice, control, and independence, while ensuring safety.
You will actively address adversity, support adults at risk of social exclusion, and assist adults in accessing support to manage adverse circumstances. You will champion equality and diversity and challenge discrimination in all its forms.
You will hold a caseload of adults with diverse mental health and social care needs. You will maintain continuous reflection and professional development, aligning with the standards set by Social Work England.
All entry-level social workers joining the Trust are expected to participate in the Assessed and Supported Year in Employment.
