We are looking to recruit a senior community nurse on a full-time basis, who is innovative and keen to join the team in supporting us to deliver high quality care, and to continue our development of integrated care in the community. The role is varied and rewarding. We are looking for an experienced passionate nurse who is excited by change and is keen to help shape a new way of delivering nursing care in the community. We are a team of health care professionals focused on compassionate care, working closely with GPs and other primary care services, drawing on the skills of our multidisciplinary team to manage complex patients and ensure our services are responsive and patient-centred.
Requirements include:

3. The ability to work autonomously, manage your own caseload, and collaborate within a multidisciplinary team
4. Strong problem-solving and time management skills
5. Effective communication skills at all levels
6. Experience working in a fast-paced and varied environment
7. Ability to travel to deliver care services
Your typical day will involve home visits for adults over 18 requiring various nursing interventions, including end-of-life care, wound management, IV therapy, and holistic assessments considering environmental and social factors. You will also participate in team meetings, care planning, evaluation, and support students and junior staff.
