Role Overview
Based at York, you will work as part of a multidisciplinary team, supporting integrated pathways across acute inpatient, outpatient and community services. You will be providing clinical leadership and expertise in relation to frailty as well as driving service developments, quality improvements and outcomes in the service. This is an opportunity to develop confidence in leading change, supporting others, and promoting best practice.
Responsibilities
* Manage a highly complex clinical caseload using advanced clinical reasoning and occupation-focused assessment.
* Lead specialist practice in frailty, ensuring safe, effective, evidence-based care.
* Act as a resource for highly specialist advice across the Trust.
* Provide leadership, supervision and mentorship to staff and students.
* Drive service development, innovation and quality improvement initiatives.
* Contribute to research, audit and evaluation activities.
* Communicate complex and sensitive information with clarity, empathy and professionalism.
* Collaborate effectively within multidisciplinary teams and across organisational boundaries.
Qualifications & Experience
The role requires advanced clinical reasoning and specialist assessment in frailty.

Additional Information
We are an Armed Forces Friendly Employer and offer a generous relocation package of up to 30% of salary (capped at £18k) for hard to fill positions. We do not offer sponsorship for roles with a salary less than £25,000 per annum. DBS checks are required for this role and will be charged to employees; the cost will be deducted from salary over one payment or over a six‑month period.
