What you will do: Screen and triage adult social care referrals to ensure effective and timely service delivery. Complete Care Act 2014 assessments and Carers Assessments, tailoring support to individual needs. Undertake Mental Capacity Assessments and make Best Interest decisions in line with legislation. Manage safeguarding screenings and Section 42 enquiries to protect vulnerable adults. Support Continuing Healthcare (CHC) processes while working within the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983 frameworks. Manage short-term caseloads, collaborating closely with families, carers, and partner agencies.
