Trafford and Manchester Case Management Service

A fantastic opportunity has arisen for enthusiastic and motivated individuals to join the pharmacy team working within Manchester and Trafford Local Care Organisation (LCO) as part of MFT.
Case management proactively supports people at high risk of hospital admission to remain at home. The service brings together the skills and expertise of GPs, nurses, mental health practitioners, social workers, pharmacists, pharmacy technicians and wellbeing advisers to provide intensive and flexible support to people who are struggling to stay healthy, safe and well.
We are looking for a General Pharmaceutical Council (GPhC) registered pharmacy technician with substantial experience of working within primary or secondary care. You will work as part of a multidisciplinary team to provide medicines support and positively contribute to patient care. You should be confident in the ability to work independently with excellent organisational and interpersonal skills. You will also need to be able to have access to a car for work purposes.
The successful candidates will become an integral part of the well supported and award-winning community medicines optimisation service (CMOS) providing a patient facing pharmaceutical service, working closely with the wider pharmacy teams and other services across MFT.

Responsibilities
* Support the delivery of medicines optimisation to the Case Management Service
* Support pharmacists to establish safe working arrangements for medicines in the Community Medicines Optimisation Service
* Assist in the development and implementation of pharmaceutical care plans
* Liaise with pharmacists, technicians and other health care professionals in secondary and primary care to ensure continuity of care on patient transfer
* Carry out medication reviews with patients in area of clinical competence
* Identify and act on medication errors/queries within area of competence, reporting complex issues to a medicines optimisation pharmacist as appropriate
* Assist in the management of prescribing across primary care
* Provide support to other CMOS workstreams as necessary
