Overview

Camden Rapid Access Services are an Adult physical health service.

Are you a Nurse or Paramedic seeking a new challenging role in the community? Camden Rapid Access Services (incorporating Urgent Community Response (UCR); Virtual Ward; and D2A pathways) are currently seeking to employ a Band 7 Registered Nurse or Paramedic with experience in Acute care (e.g. A&E; Paramedic; ITU; CCU; HDU).

Potential post holders must possess current, valid NMC (UK) or HCPC (UK) registration.


Responsibilities

* Urgent Community Response: provide high-quality care to patients in the community experiencing acute deterioration in health that could otherwise lead to hospital admission; assess, manage, and treat patients in their homes or care settings; aim to review patients within two hours of referral; play a crucial role in ensuring timely clinical support and reducing strain on emergency services.
* Virtual Ward: non-medically optimised patients admitted as an early discharge from local acute trusts or escalated from the Rapid Response Team; care overseen by a Consultant Geriatrician; focus on elderly care with a mix of conditions including IV medication, remote monitoring using high-spec technology, and management of acute/chronic medical conditions.
* Shifts: work patterns including 08:00–21:00 (long day), 09:00–22:00 (long day), 09:00–19:00 (early), and 12:00–22:00 (late); two out of four weekends per month; up to 4 shifts or specific days off per roster.
* Collaborate with multidisciplinary teams and contribute to a wide range of patient visits in the community alongside nurses, paramedics, occupational therapists and physiotherapists.


Requirements and Qualifications

* Current, valid NMC (UK) or HCPC (UK) registration.
* Within the team, we have Registered Nurses; Paramedics; Occupational Therapists and Physiotherapists; work with OT/PT colleagues and joint visits as needed.
* Completion of Physical Assessment Module (or suitable alternative such as the A&E; ITU or DN Course) is required for non-Paramedic applicants; Paramedics are exempt as it is part of the basic curriculum for student paramedics.
* Full UK Driving licence preferred; a car is provided for use on some shifts; bus/tube passes can be provided if no licence; location covers the entire borough of Camden.


Skills

* Completion of Physical Assessment (Essential) and Venepuncture with ability to identify required blood tests
* Cannulation; Medication Administration (PO; PR; IV; IM; SC etc); Triage; Autonomous Practice
* Confidence in clinical knowledge; awareness of Mental Capacity Act and Mental Capacity Assessment
* Proficient at taking on a Nurse/Paramedic in Charge role; Non-Medical Prescriber (Desirable); Teaching, Mentor or Practice Assessor skills
