Overview
Camden Rapid Access Services are an Adult physical health service. Are you a Nurse or Paramedic seeking a new challenging role in the community? Camden Rapid Access Services (incorporating Urgent Community Response (UCR); Virtual Ward; and D2A pathways) are currently seeking to employ a Band 7 Registered Nurse or Paramedic with experience in Acute care (e.g. A&E; Paramedic; ITU; CCU; HDU). Potential post holders must possess current, valid NMC (UK) or HCPC (UK) registration.
Responsibilities
* Urgent Community Response: Nursing / Paramedic Led Caseload; provide high-quality care to patients in the community experiencing acute deterioration, assess, manage and treat in patients\' own homes or care settings, aim to review within two hours of referral, and reduce unnecessary hospital admissions.
* Virtual Ward: Consultant Led Pathway for non-medically optimised patients admitted for early discharge or escalation; focus on elderly care with conditions requiring IV medication, remote monitoring and management of acute/chronic conditions.
* Shifts: 08:00-21:00; 09:00-22:00; 09:00-19:00; 12:00-22:00; two out of four weekends per month; up to 4 shifts or specific days off per roster.
Requirements
Registration: current NMC (UK) or HCPC (UK). Driving: Full UK Driving licence preferred; car may be provided for some shifts. You should be forward thinking, able to prioritise based on clinical need, and act as a Senior Practitioner when needed.
Qualifications
Essential
* Registered Nurse with current NMC registration or Registered Paramedic with current HCPC registration
* Bachelor\'s Degree (BSc) in Nursing/Paramedic Science
* Evidence of continued professional development
* Mentorship (Post Grad Module) or Practice Assessor / Supervisor Training
* Advanced Clinical Assessment Course

* Master\'s or working towards
* Independent Nurse Prescriber
Experience
Essential
* Extensive post registration experience in Acute care including ITU, A&E, Acute assessment units and/or Rapid Response
* Evidence of working within the MDT
* Understanding of primary care role in admission prevention and timely discharge

* Phlebotomy, IV Cannulation and IV therapy skills (including PICC and Central line) or willingness to learn
* Excellent clinical reasoning and decision making

* Ability to interpret blood results
