Clinical Duties Delivering high quality, compassionate care for older & vulnerable people living at home either alone or with their family so that they receive proactive care that steers them away from avoidable unscheduled care. To provide a continuously improving service in line with the clinical governance framework of The Primary Care Network Practices as well as the local and national drivers of older peoples services. The post holder will work as a member of the PCN complex care team but will need to be flexible to the needs of given practices within our network. Undertake clinical tasks when required.
Play an integral role in the delivery of vaccination campaigns. To assess, plan, implement and evaluate care plans for patients; promoting proactive independence and autonomy; working within a multi-disciplinary team. To work in collaboration with other professionals to deliver and ensure care is delivered appropriately. To demonstrate clinical effectiveness of service delivery by use of evidence-based practice and outcome measures.
To have the skills to take a history, assess, examine, diagnose, and develop a management plan. To recognise, assess and manage risk across the immediate and wider working environment and make appropriate decisions autonomously ensuring statutory requirements are met. To be responsible for patient safety through knowledge of systems, legal requirements and understanding of litigation. To communicate effectively verbally and in written form in the exchange of highly complex, sensitive, or contentious information in demanding situations using de-escalation, mediation, resolution, and professional Duty of Candour.
To evaluate care, taking appropriate action leading to improvement in quality standards through clinical audit, root cause analysis and dealing with complaints. Personal Development Take ownership of own development pathway (with appropriate support).
