Children's Community Nurse/ Continence lead - Band 6.

There is an exciting opportunity for a Band 6 Paediatric Continence nurse/Children Community nurse to join the Children's Community Nursing team (CCNT). The post holder will have continuing responsibility for the management of a defined Child / Young Person Caseload and delivery of Specialist care for patients who have been referred to the Paediatric Continence Service across SBUHB

They will be responsible for the continuity of care.

The primary function of the role is to provide proactive, responsive services that address the anticipatory and maintenance care needs of the patients/clients within the defined caseload.

This will be done via a holistic assessment of care needs and the provision of clinical care for patients/clients with additional & complex needs.

Practice autonomously and demonstrate evidence based clinical decision making.

Ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes.

The successful candidate will provide leadership, professional advice and care for the Community Children’s Service in SBUHB.

The post holder will work in partnership with the appropriate multi-disciplinary agencies to provide skilled nursing care to children and young people.

The post holder will be expected to work across Swansea and Neath Port Talbot areas according to service needs.

Main duties of the job

To facilitate the provision of effective, efficient and appropriate high quality care to Community Children’s Nursing Service ensuring the highest standard of nursing care is provided, monitored and evaluated.

To support the Community Children’s Nursing Team in assessing, planning, implementing and valuating the holistic nursing and health care needs of children and young people with a range of conditions from acute illness to complex, life limiting and end of life care.

Provide information to the Continence service and CCN team Manager Matron on patient activity as required to promote effective service management

Essential qualifications and knowledge

Essential criteria

1. Relevant proven paediatric community experience.
2. Able to proficiently organise a load of cases.
3. Evidence of the ability to work in partnership with other agencies and Families.
4. Evidence of reflective practice.

Desirable criteria

5. PGCE or Teaching & Assessing module.
6. CCN SPQ qualification

Essential Aptitude and abilities

Essential criteria

7. Specialist knowledge across the range of Community Children’s Nursing and practices
8. Excellent communication skills – verbal and written.
9. Leadership skills and the ability to respond to change.
10. Motivate staff and be able to contribute to the development of others.

Desirable criteria

11. Welsh Language Skills are desirable levels 1 to 5
12. Negotiating skills.
