About The Role

At Alzheimer’s Society, we believe no one should face dementia alone. Every conversation, every visit and every small moment of reassurance can make a real difference. We are looking for a warm, thoughtful and organised Dementia Adviser to join our supportive Milton Keynes team.

You will offer support to your clients in a variety of ways, whether in the client's own home, or at other locations in the community, face to face, by phone, letter, or email. You will also assist service users to access other services, providing signposting and referrals, and facilitating peer support groups for people living with dementia on occasions in various location in the area!

You will have your own local patch, walking alongside people living with dementia and the people who care about them. This is a role built on trust and kindness. You will listen carefully, understand what matters most to each person and offer guidance that helps them feel steadier and more confident as their situation changes.

You will be part of a caring and dynamic local team who share ideas, reflect together and support one another. There will be time to meet as a team and opportunities to keep growing your knowledge of dementia, with your manager and colleagues alongside you.

Hours - 35 hours
Location - Community based in Milton Keynes (or within a commutable distance) with some homeworking. Interviews: provisionally scheduled to take place via MS teams on 8th & 10th April.

About you

You will have:

1. An understanding of dementia, or a willingness to learn about the experiences of people living with dementia and those who care for them.
2. Experience providing information, guidance, advice or emotional support - this could be from health, social care, community, voluntary, customer service or other people focused roles.
3. You will be able to listen and communicate, with the ability to build trust and rapport with people from a wide range of backgrounds.
4. The ability to manage your workload effectively, with appropriate support and tools, and to balance competing priorities.
5. Confidence using IT systems to maintain accurate and confidential records.
6. A compassionate, non-judgemental and person-centred approach, with respect for diversity and individual lived experiences.

Key Responsibilities:

7. Your primary role will be in delivering personalised and tailored support to people affected by dementia.
8. Build supportive, trusting relationships with people living with dementia and their carers across, recognising and respecting individual needs and circumstances.
9. Provide personalised information, advice and emotional support that reflects each person’s culture, background, values and preferences.
10. Deliver support through home visits, telephone contact and partnership working with professionals.
11. Manage a defined caseload, maintaining accurate, respectful and confidential records.
12. Referring people to the appropriate health and social care organisations when required,
13. Signpost and connect individuals to appropriate local services and community networks, reducing isolation and increasing access to support.
14. Providing advice and guidance at local peer groups, specific dementia groups and community networks, whilst looking out for new opportunities to promote the service and maintain the profile of dementia services across the area.
