Work collaboratively with citizens, partner agencies, and community services to support adults with disabilities in Powys.
Practice under current legislation, including the Social Services and Wellbeing Act.

What you will do:
• Assess and manage complex needs of adults with disabilities and their carers.
• Liaise with other services (e.g., Reablement) for seamless care pathways.
• Promote partnership working and person-centred outcomes.
• Undertake safeguarding investigations as required.
• Maintain a manageable but complex caseload.
• Promote social inclusion, equal opportunities, and respect for diversity.
• Prepare reports, maintain accurate records, and ensure statutory compliance.
• Engage in direct work, holistic assessments, and care planning.
• Undertake Mental Capacity Act assessments and Best Interests decisions.
• Contribute to professional development of others (mentoring, training).
• Attend interagency meetings and coordinate care reviews.
• Understand budget/resource implications and encourage positive risk-taking.
