Central Cheshire Integrated Care Partnership (CCICP) is expanding its team of General Nursing Assistants (GNAs) who work as part of the Integrated Placement of Care Hub. GNAs provide care and support to patients at home. Examples include assisting with personal care, medication administration, preparing light meals, assist with catheters or stomas, and supporting rehabilitation exercises.

CCICP is looking for people with experience in care settings or as a Health Care Assistant, with a commitment to high quality patient care. GNAs work across CCICP's five care community hubs: Nantwich, Northwich, Winsford, Crewe & SMASH (Sandbach, Middlewich, Alsager, Scholar Green and Haslington).

Shifts are typically 8am – 4pm and/or 2pm – 10pm, across 7 days a week. Full and part-time roles are available with full training provided. Opportunities for career progression are available and the role provides experience for anyone looking to pursue a career in nursing in the future.

Main duties of the job

As a General Nursing Assistant (GNA), you will work as part of the Integrated Placement of Care Hub (IPOCH). The service focuses on a “home first” approach, rehabilitation and promotion of independence for patients leaving acute/bed-based settings or requiring step up care in their own homes. A significant part of the role involves supporting end of life patients in their own homes, providing a dignified and high-quality approach to care for patients and families during challenging times.

You will work under the direction and with full support from wider care community teams, including district nurses, therapy teams and advanced community practitioners. The role includes day-to-day training from a dedicated GNA trainer and mentoring from a band 6 nurse, with ongoing appraisals and one-to-one meetings. You will have support from senior GNA practitioners and a focus on developing the skills needed to provide excellent care.

You will undertake record keeping using an electronic device, for which training will be provided. Applicants must hold a valid UK driving licence.


Job responsibilities

* To work under the direct supervision and guidance of the nursing team.
* Act on own initiative in delivering patient care in the community.
* Support multidisciplinary health professionals in providing a rehabilitation programme to prevent avoidable hospital admission.
* Provide care and rehabilitation for service users within their own homes under clinical direction, including assessing patients’ condition, recording on EMIS, and informing relevant professionals in a timely manner.
* Communicate factual information to patients/carers with patience, persuasion and reassurance.
* Provide high quality care for patients and families within the community setting, including end of life care.
* Manage occasional distressing or emotional circumstances and careful use of equipment; ensure hoists and equipment are safe and properly used.
* Maintain accurate personal and professional records in line with trust policies.
* Support patients with administration of medications.
* Utilise resources efficiently and cost-effectively.
* Report accidents/incidents and complete appropriate forms promptly.


Person Specification


Qualifications

* Previous experience of working in the NHS and/or a nursing/caring environment.
* Ability to use own initiative when required.
* NVQ level 3 in health care or equivalent, or willingness to work towards.
* Understanding of the NHS and current healthcare issues.
* Experience of providing rehabilitation to others.
* Experience of working within a multidisciplinary team.
* Experience of working with end of life patients.


Skills

* Excellent interpersonal skills; good verbal communication; IT literate with documentation skills; organized and able to manage time effectively.
* Ability to work with IT systems; experience of working with patients who have difficulty communicating.


Knowledge

* A commitment to quality and safety; respect, dignity and compassion; willingness to listen, learn and lead; and a commitment to work with others to achieve the best outcomes.
* Evidence of role modelling, problem solving and contributing to service development; support for self and colleagues to strive for improvements.

Physical requirements

* Occupational Health clearance to perform duties; ability to perform a wide range of duties; valid driving license; CRB/DBS check as required.
