Job summary
An exciting opportunity has arisen to join the IBD Nurse Specialist team. We are a small team that works closely together for the benefit of IBD patients. We are looking to welcome a highly motivated, confident and enthusiastic nurse who will be an integral part of our supportive IBD nursing service. The successful applicant will work closely with the IBD Advanced Clinical Practitioner and the MDT to provide excellent, specialised nursing care and support for patients with IBD in inpatient and outpatient clinic settings.
Responsibilities
* Accept Patient referrals to agreed criteria and be identified as the key worker for an agreed group of IBD patients.
* Provide professional and clinical leadership in the care of patients with IBD.
* Assist in providing a high-quality Inflammatory Bowel Disease service throughout the hospital.
* Assess and educate patients with Inflammatory Bowel Disease.
* Provide information and advice to ward nurses.
* Assist in maintaining the IBD helpline by being a first point of contact for patients in the community.
* Act as a resource to patients, carers and other health care professionals within the hospitals and community by providing expert knowledge and advice.
* Communicate effectively as an integral member of the multidisciplinary team to achieve a co-ordinated service across the Trust.
* Provide education and support the development of other staff members.
* Assist in the development of clinical services and enhanced practice across the Trust.
* Lead a specialist area of practice, providing expert professional advice to others across the Trust within agreed guidelines and protocols, including medical colleagues and other professional groups when required.
* Perform comprehensive specialist assessment of patients health and psychosocial needs, plan, commence, treat, and evaluate care within agreed guidelines and protocols according to evidence-based practice and changing health needs.
* Communicate complex specialist information to medical colleagues and others. Refer patients to appropriate clinicians, in the Trust or externally, within agreed guidelines and protocols, according to assessment of patient need and evidence-based decision.
* Prioritise patient care, supervising and allocating one's own workload, monitoring, and evaluating effectiveness of care, including discharge of patients, or referring on, within agreed guidelines and protocols.
* Perform specialist clinical skills in the treatment and overall management of patient care, including requesting and interpreting investigations, within agreed guidelines and protocols.
* Provide a comprehensive specialist service, accepting accountability and responsibility for the management and effectiveness of the service on a day-to-day basis.
* Collect, collate, evaluate, and report on information, including communicating detailed information to clinical colleagues and others, maintaining accurate patient records and documentation.
* Promote patient and public involvement in the improvement of services and involve patients and carers/relative in the planning and delivery of their care.
* Work collaboratively with other professionals and agencies to ensure patient needs are met, especially in relation to patient flow, care pathways and discharge arrangements.
* Analyse and act on information to proactively manage and maintain NHS targets and standards.

Qualifications and Skills
Essential
* RGN
* 1st Degree/equivalent experience
* Broad range of clinical experience, post registration experience at a senior level
* Recent evidence of study in speciality or related field
Desirable
* Nurse prescribing desirable
* Gastro/IBD Experience
