Job summary
CommunityUrgent Care Team is a 24/7 service; providing a first point of contact andcomprehensive triage service, telephone and clinical assessment to patientsand/or their carers/relatives resulting in a safe and appropriate clinicaldecision and plan of care.
The team is responsible for health,therapy and social care triage including supporting aversion of hospitaladmission or early hospital discharge through assessment and treatment ofindividuals in crisis, assessing complex and specialist needs in order todevelop, implement and evaluate programmes of care. Supporting individuals to remain in communitysettings, in their own home, residential home or nursing home.

Salary Range 7£41,617 - £47,499
The postholder will provide a high standard of OccupationalTherapy service undertaking all aspects of clinical duties as an autonomouspractitioner.
You will manage a defined caseload of patients using evidence-based practice andmodels of practice to provide a patient centred assessment and to plan,implement and evaluate interventions, including discharge planning.
You will promote the importance of Occupational therapy within servicesincluding the wider MDT and external organisations .
You will also liaise with the wider MDT, supervise, educate and assessoccupational therapy students, assistants and junior occupational therapistsand provide specialised advice to other health care professionals in thespecific clinical area.
Undertake evidence-based audit and research projects to further own andteams clinical practice within each rotation, making recommendations to Clinical Lead/Manager of service for changes to practice by the team. You may lead theimplementation of specific changes to practice or contribute to development ofservice protocols.

3. Wide range of CPD activities related, which may include studies up to Masters level, related to the relevant specialist area
