Overview

NHSGGC is one of the largest healthcare systems in the United Kingdom, employing approximately 40,000 staff across clinical and non-clinical roles. We deliver acute hospital, primary, community and mental health services to a population of over 1.15 million people, with 2.2 million when regional and national services are included.

The Role

An exciting opportunity has arisen within the NHS Greater Glasgow and Clyde Learning Disability Health Check Team for a Band 5 Staff Nurse to support the implementation of Learning Disability Health Checks. This position is fixed-term for 18 months. The team is a Nurse-Led service implementing the Scottish Government Directive “The Annual Health Checks For People With Learning Disabilities (Scotland) Directions 2022”.

You will be responsible for undertaking health checks within the board area, both in clinic and community environments, as well as in individuals’ own homes. As part of the health check, you will determine appropriate recommendations and referrals based on your assessment and up-to-date evidence-based practice. You will work with wider primary care and learning disability services to reduce barriers to accessing health services for this population group. You should be experienced in working with individuals with learning disabilities and have excellent knowledge of their health needs.

Qualifications must include:

You need to have a live NMC UK pin in order to be considered for this post. Must have a FULL UK DRIVING LICENSE.

Duration, Location, and Working Pattern

* Location-based in Barrhead Health & Care Centre
* Fixed-term Contract for 18 months
* Full-time position (36 hours per week)
* Shift pattern: Monday-Thursday 08:30-16:30; Friday 08:30-15:00


Key Responsibilities

* Have continuing overall responsibility for the delivery of health checks across NHS GGC.
* Provide summary reports of health check assessments.
* Be responsible for the specialist clinical nursing assessment, utilising the health check tool for individuals with a learning disability, providing person-centred care.
* Provide clinical knowledge and skills in promoting and maintaining optimum health for adults with learning disabilities with co-morbidity.
* Work in partnership with clients, carers and other care providers, promoting the principles of social inclusion.
* To work within the framework of relevant legislation (e.g., Adults with Incapacity, Mental Health (Care & Treatment) (Scotland) Act 2003) and ensure up-to-date certificates of incapacity where applicable.
* Contribute to high-quality, person-centred care by developing and implementing standards of care and evidence-based nursing interventions based on national guidance and standards.
* Operate within a clinical governance framework and collaborate with multi-agency teams to maintain high standards of care.
* Adhere to legal and ethical standards relating to nursing and learning disability-related legislation and ensure the well-being of people with learning disabilities.
* Promote the public health role of community learning disability nurses, identify health needs and support health improvement activity.
* Maintain confidentiality in line with Nursing and Midwifery Council policy.
* Establish and maintain effective communication within a multi-agency framework and maintain contemporaneous written records and reports as required.


Knowledge, training, qualifications and/or experience

* Registered Nurse 1st level RNLD or equivalent.
* Professional/clinical knowledge at degree level.
* Relevant post-registration experience.
* Evidence of post-registration educational development.
* Flexibility and adaptability in responding to service needs in a dynamic environment.
* Excellent organisational and interpersonal skills.
* Knowledge of current law and policy trends in learning disability and community care.
* Excellent training and presentation skills.
* Ability to view people with learning disabilities as equal citizens.
* A full GB driving licence.
