We are looking for a proactive and innovative individual to join our growing, patient‑focused primary care team as a Practice‑Based Care Coordinator. This is an exciting opportunity to contribute to a developing service that places patients at the heart of everything we do.
This role is based across two sites in West Bridgford: West Bridgford Medical Centre and Castle Healthcare. You will play a key part in supporting both patients and clinical teams by coordinating care, managing data, and contributing to a wider range of practice initiatives.
Main duties of the job
* Run, analyse, and interpret reports from the clinical system (SystmOne), ensuring patient records are accurate and up to date.
* Coordinate and book patient appointments, supporting smooth patient pathways.
* Work closely with GP practice teams to provide essential administrative and coordination support.
* Support clinical initiatives and help patients manage their care by providing clear, accessible information.
* Support the use of digital health tools.
* Maintain and update practice websites.
* Use clinical systems for record‑keeping, audit, and evaluation, ensuring compliance with organisational policies.
* Contribute to service development projects and NHS initiatives, including vaccination programmes.
* Work independently, manage your own workload effectively, and seek guidance when appropriate.
Additional responsibilities
* Run reports from clinical systems such as SystmOne (training will be provided).
* Keep patient clinical records accurate and up to date using read coding and data entry.
* Generate reports to analyse appointment data for capacity and demand.
* Support patients’ appointment booking and provide clear information for choices.
* Manage clinics, including staff rosters, when required.
* Facilitate self‑management education courses, peer support and interventions.
* Coordinate multidisciplinary meetings.
* Promote and support access to NHS App & Patients Know Best for patients, community & colleagues.
* Maintain PCN and GP Practice websites and waiting room screens (Envisage).
* Create quarterly and annual business reports using clinical reporting and digital editing tools (Canva).
* Adhere to organisational policies and procedures, occupational policies in primary and secondary care.
* Maintain confidentiality at all times.

* Experience working within GP practice and healthcare.
* Proficiency in Microsoft Office (Word, Excel) and general software applications.
* Good understanding of confidentiality and GDPR.
* Strong problem‑solving, analytical and resourceful skills.
* Integrity, sound professional judgement and self‑motivation.
* Ability to prioritise tasks, meet deadlines and adapt to changing needs.

* Driver with own vehicle required.
