Hospice Community Paramedic Practitioner (development post)
JOB SUMMARY: To provide specialist palliative care to patients and their families within a service that operates every day and at all hours, by phone, visits, and advising and teaching others. The role offers both proactive and reactive clinical support for adults living with a progressive illness.
CLINICAL RESPONSIBILITIES: To work within a team providing specialist palliative care in community settings, including the hospice, community units, and care homes. Support carers and other care providers through the advice line, training, and education. Conduct holistic assessments of patients with palliative needs, regardless of diagnosis, and plan care accordingly. Provide information, advice, and psychological and spiritual support to patients, families, and carers, liaising with healthcare professionals as needed. Support admission prevention and facilitate timely admissions for reversible conditions. Visit patients and families when specialist consultation is necessary or see patients in the Community Unit. Offer telephonic support to patients, families, and professionals day and night. Participate in multidisciplinary meetings and present patient cases. Provide urgent care interventions as needed to improve patient comfort. Represent the hospice at meetings and ensure effective communication among all involved in patient care. Offer support and expertise to professionals, acting as a role model and resource. Supervise junior staff, oversee workload delegation, and conduct annual appraisals. Comply with drug policies, including Non-Medical Prescribing, and understand safeguarding principles.
MANAGEMENT RESPONSIBILITIES: Collaborate with the Community Team Leader to plan and implement service improvements. Ensure service coverage during absences and high demand. Manage patient caseloads, maintain records, and participate in education and training. Lead clinical standards development, research, and audits within the Clinical Governance Framework. Record equipment loans and ensure proper management. Make autonomous decisions while consulting colleagues as needed. Deputise for the Community Team Leader when required.
PERSONNEL AND EDUCATIONAL RESPONSIBILITIES: Participate in staff placements and training, recognize staff stress, and facilitate support. Identify personal educational needs and develop a professional development plan. Update skills through continuous learning, participate in education programs, and contribute feedback. Engage in supervision and peer support.
PERSONAL AND ORGANISATIONAL DEVELOPMENT: Devise a personal development plan aligned with hospice objectives. Maintain and enhance clinical and managerial skills. Work within hospice policies, ensure resource efficiency, and uphold confidentiality. Promote hospice values and foster good relationships with all stakeholders.
