Join to apply for the Deputy Sister/Charge Nurse, Frailty OPAU role at Northumbria Healthcare NHS Foundation Trust

An exciting opportunity has arisen for 12 month secondment for a Band 6 Frailty Nurse to join our existing frailty team at ASPH Older Persons Assessment Unit.

Our multidisciplinary Frailty Team delivers person centred care, frailty screening and comprehensive assessment and care planning for older adults with frailty admitted to the ASPH.

This post will be of potential interest to nurses who are working at Band 5 or 6 who have an interest and experience in working with older people. The successful candidate will have an understanding of frailty and the comprehensive geriatric assessment and a wish to progress and apply their expertise in a stimulating clinical environment.

We can offer strong support for development within our expert team, with help to develop your skills and knowledge as well as opportunities to take part in shaping the future of the frailty service and to deliver teaching and education on frailty for The Trust.

Ashford and St. Peters Hospitals NHS Foundation Trust serves a population of more than 410,000 people living in North-West Surrey, parts of Hounslow and beyond.

Over 3,700 highly trained doctors, nurses, midwives, therapists, healthcare scientists and other support staff make up our workforce, providing a wide range of services across our two hospital sites, Ashford, Surrey and St Peter's, Chertsey, Surrey.

We also run many specialist clinics in the community and local community hospitals and other healthcare facilities.

Our vision is to be one of the best healthcare Trusts in the country. There has never been a better time to join us in the NHS at ASPH. We are committed to providing continuous professional development, staff benefits and flexibility to shape our workforce around our patient care.

This is an exciting time to join our Trust, as we work on the expansion of our theatres at Ashford Hospital and move towards this becoming our dedicated elective centre. We want to create a state-of-the-art centre for excellence for planned surgical procedures; with care delivered by dedicated, specialist teams in a modern and well-equipped environment.

We hope to use this centre to create an environment of learning and to facilitate training of the next generation of our skilled NHS workforce. Our plans are ambitious - why not, come and be a part of our journey.

Adverts may close early, so applicants are encouraged to submit an application as soon as possible.

This post forms part of the Frailty Team, working within Older Peoples Services at ASPH. The team includes doctors, specialist nurses and therapists and provides specialist care for frail and elderly patients in the Emergency Department and acute assessment areas.

The Frailty Nurse will be able to support patients who meet the clinical criteria for frailty and provide a constructive care plan to enable an early discharge into the community, avoiding unnecessary admission into hospital. The post holder will have a delegated responsibility to accurately assess and screen patients in a timely and efficient manner.

The post holder will provide an assessment of care needs, development, implementation and evaluation of care to provide clinical support to patients, families/carers.
