Health & Social Care Officer – Front Door Team (Adult Social Care) We are seeking a dedicated and confident Health & Social Care Officer to join a busy Front Door service supporting Adult Social Care referrals. This is a vital role acting as the first point of contact for vulnerable adults who may have complex, sensitive, or rapidly changing needs. You will play a key part in ensuring that every referral is triaged efficiently, accurately, and with a person-centred approach, helping individuals access the right support at the right time. The Role Working within a fast-paced safeguarding and assessment environment, you will be responsible for: Receiving, processing, and triaging incoming adult social care referrals Handling safeguarding concerns, care support requests, and occupational therapy referrals Ensuring timely and accurate recording of information across systems Making informed initial decisions on prioritisation and onward pathways Liaising with internal teams and external partners to coordinate appropriate responses Supporting the delivery of an effective, responsive front door service for adults with a range of needs Managing situations that can be complex, unpredictable, and sometimes high-pressure About You We are looking for someone who: Has a strong understanding of adult social care services and pathways Can confidently assess and prioritise information in a fast-moving environment Communicates clearly and compassionately, even in challenging situations Is confident working with vulnerable adults, including those with complex needs Works well both independently and as part of a multidisciplinary team Has experience in social care, safeguarding, referral management, or similar settings Working Arrangements This is a hybrid role, offering a 50/50 split between office and home working. Please note: an initial full-time office-based period will be required to ensure full training, confidence, and understanding of processes before transitioning to hybrid working.
