Job Title: Personal Service Coordinator/Intensive Case Manager
Hours and Compensation: 35 hours per week / $24.00 per hour (actual salary based on experience and other factors)

Position Summary:
Provide high-quality Intensive Case Management Services (ICMS) to clients eligible for permanent housing through the DHS Housing for Health (HFH) program. Focus on a client-centered approach and excellent customer service, addressing the challenges faced by people experiencing homelessness with medical and behavioral health issues as they move into and maintain permanent supportive housing.
Essential Duties and Responsibilities:
* Assist individuals throughout the housing stabilization process, including documentation, interviews, and subsidy management related to LACDA/HACLA programs.
* Conduct outreach and engagement, process referrals, establish rapport, perform face-to-face screenings, obtain consents, and link clients to needed services.
* Manage intake and enrollment, assist with documentation, and support clients in securing temporary and permanent housing.
* Support clients during move-in, help with applications, and orient tenants to their new housing.
* Develop and implement individualized case management plans based on comprehensive assessments, addressing clients' needs and goals.
* Ensure linkage to health, mental health, and substance use services, and assist with medication management and establishing medical homes.
* Assist clients in accessing income, benefits, employment, education, and community activities; support daily living skills and barrier removal.
* Provide housing location services, educate on tenant rights, and assist with eviction prevention and resolving housing issues.
* Maintain accurate documentation in accordance with organizational and funding standards, including timely records in Electronic Health Records.
* Participate in on-call crisis prevention rotations and promote crisis intervention awareness.
* Engage in ongoing professional development to enhance skills and knowledge.
Other Requirements:
* High school diploma or GED; 2 years’ experience in psychosocial rehabilitation services.
* Team-oriented, proficient in MS Office, and able to learn HMIS and EHR systems.
* Experience working with clients with mental illness, chronic health issues, and substance use disorders.
* Knowledge in homelessness, outreach, case management, crisis intervention, housing rights, and benefits.
* Flexible schedule including nights, weekends, and holidays.
* Valid Class C driver’s license, vehicle, insurance, TB clearance, and ability to meet employment conditions.
